Every now and then, a medical textbook of rare clarity and insight is published that makes the reader ask "why didn't anyone think of this before?". Drs. Shah and Ludwig have done just that with this title. The structure of the book is simple: each chapter deals with a specific pediatric chief complaint. Within each chapter there are four to eight case presentations that guide the reader through a structured, logical, and illustrative discussion of the workup of each sign or symptom.
The result is a book that teaches the reader how to think about broad clinical questions such as dark urine, poor weight gain, or wheezing in a child. By the end of each chapter, the reader should have a basic understanding of how to approach each problem and, more importantly, when to suspect a rare condition.
Practitioners in any setting, whether inpatient, outpatient, or emergency room, will certainly find this textbook a useful learning tool.

As a newly minted FNP, this book has been my casual reading for several weeks. The cases are engaging, the discussion is enlivening and the clinical pearls are numerous. I carry several of these cases in my mind whenever I see patients.
